AGE CALCULATOR

The Chronological Age Calculator is designed to assist schools in determining a student's chronological age. To show and print an age table, select a school year and a school month below. A new window will open that displays the age table based from the selected school year and month. To find the age of a student, select the student's birth year and month, and enrollment year and month. Click on Calculate to compute for the age. ENROLLMENT INFORMATION

Enrollment Requirements

To receive an enrollment package, you must live in the Rosemont Avenue Elementary attendance area or be granted a valid permit (subject to space availability and applicable approvals).  You must provide the following documents:

     1. Your Proof of Residence (see below)

     2. Your Driver's License, ID Card, or Court paper of Affidavit of Parental/Guardian Identification, or Caregiver Affidavit

     3. Student's Original birth certificate, baptismal certificate, passport, court order, health office/vital statistics record of birth certificate date, or letter from the Department of Public Social Services (DPSS) verifying the birthdate and an explanation of how this was verified

     4. Student's immunization record (see below)

 

To show Proof of Residence, bring one of the following documents (REF-6554.4):

  • Most recent utility service contract, bills or payment receipts (gas, water, electric)
  • Property tax payment receipts
  • Rental/lease agreement or current rental receipt with address of property on receipt
  • Official government mail (CalWorks, Social Security, Medi-Cal)
  • Current pay stub
  • Voter's Registration

(Note: LAUSD does not accept telephone bills, driver's license or DMV Identification Cards as proof of residence.)

 

Immunization Requirements (Bul-1660.8): 

California School Immunization Law

  • POLIO: 4 doses, but 3 doses approved for ages 4-6, if at least one was given after the fourth birthday; 3 doses approved for ages 7-17 years if at least one was given on or after second birthday
  • DTP: 5 doses, but 4 doses meets requirement for ages 4-6, if at least one was given on or after the fourth birthday; 4 doses, but 3 doses approved for ages 7-17, if at least one was on or after second birthday. 
  • MMR or MMR-V: Ages 4-6 years old, 2 doses (both doses on or after first birthday); Ages 7-17 years old, 1 dose on or after first birthday
  • Hepatitis B: 3 doses
  • Varicella (Chicken Pox): 1 dose required
